Factores implicados en el fracaso escolar de adolescentes con conducta delictivaColombia se encuentra en proceso de implementación del servicio educativo del Sistema de Responsabilidad para Adolescentes y en este contexto, el presente artículo describe las características académicas de adolescentes con conducta delictiva, determina las relaciones que se presentan entre los factores académicos y analiza cuáles tienen efecto en el fracaso escolar. Para ello se desarrolló un estudio predictivo transversal en el que participaron 248 adolescentes de Bogotá y Cundinamarca, 85.5% chicos y 14.5% chicas, entre los 14 y 20 años (M=16.72, D.E.=1.128). Los resultados evidencian altos niveles de fracaso académico y relaciones moderadas entre los factores académicos con el fracaso escolar. Las variables académicas que mejor ayudan a predecir el fracaso escolar fueron el bajo desempeño y los déficits en las oportunidades de participación escolar, las recompensas por participación escolar y las habilidades sociales. Las implicaciones de estos hallazgos son un tema de discusión.

Background: Recent metanalysis reported a higher risk of postoperative bleeding from pancreaticogastrostomy (PG) reconstruction following pancreaticoduodenectomy (PD). We aimed to evaluate incidence, risk factors and outcomes of hemorrhage from PG in a large single center experience. Methods: We retrospectively evaluated all consecutive PDs performed over 11 years. PG reconstruction was performed according the double pursestring invaginated technique. Multivariate logistic regression analysis was performed to identify risk factors for PG bleeding. Results: During the study period, 695 consecutive patients underwent PD with PG. The majority were performed for ductal pancreatic adenocarcinomas (410, 59%). Simultaneous venous resection was performed in 321 patients (46.1%) while arterial resection was performed in 111 patients. Postoperative 90 days mortality was 4.75% (n=33). Bleeding form PG occurred in 33 patients (4.7%) at a median interval of 5 days from surgery (0-14) leading to reoperation in 22 patients (66.6%). PG bleeding-related mortality was 9.0% (3/33). Multivariate analyses identified soft pancreatic consistency and wirsung duct > or > 3 mm (Class C and D of the ISGPS (HR=3.92, CI 95%,1.01-4.83 ;p=0.01) and the wrapping of the invaginated pancreas by haemostatic material (HR=0.37, CI 95%,0.15-0.87 ;p=0.04) as independent risk factors for PG bleeding. Conclusions: In a large volume setting, postoperative bleeding from PG reconstruction occurred in about 5% of patients. Related mortality and reoperation remain high indicating the need for intensive postoperative surveillance mainly in patients with soft pancreatic parenchyma undergoing PG. Wrapping of the invaginated pancreatic stump might reduce the incidence of hemorrhagic complications.

Low grip strength and gait speed as markers of dependence regarding basic activities of daily living: the FIBRA study2024 •
Objective: To determine whether low muscle strength and low gait speed increase the risk of disability related to basic activities of daily living in community-dwelling older adults. Methods: A longitudinal study (9 years of follow-up) was conducted with 390 older adults who were independent in basic activities of daily living at baseline and answered the Katz Index at follow-up. Associations were determined using Pearson’s χ2 test with a 5% significance level and logistic regression analysis. Results: Increases in prevalence between baseline and follow-up were observed for low muscle strength (17.5%-38.2%), slow gait speed (26.0%-81.1%), and functional dependence (10.8%-26.6%). At follow-up, 11.7% of the participants had concomitant functional dependence, slow gait speed, and low muscle strength. Slow gait speed remained a predictor of dependence in basic activities of daily living (odds ratio=1.90; 95% confidence interval=1.06-3.41). Conclusion: Slow gait speed is a predictor of functional dependence, constituting an important variable for screening functional decline.

Background In the last decade academic stress and its mental health implications amongst university students has become a global topic. The use of valid and theoretically-grounded measures of academic stress in university settings is crucial. The aim of this study was to examine the factorial structure, reliability and measurement invariance of the short student version of the effort-reward imbalance questionnaire (ERI-SQ). Methods A total of 6448 Italian university students participated in an online cross-sectional survey. The factorial structure was investigated using exploratory factor analysis and confirmatory factor analysis. Finally, the measurement invariance of the ERI-SQ was investigated. Results Results from explorative and confirmatory factor analyses showed acceptable fits for the Italian version of the ERI-SQ.
